ABSTRACT

Objective:To develop the supportive care information platform based on full cycle needs of patients and explore the application effect of platform in hemiplegia patients with ischemic stroke. Methods:Based on the supportive care scheme for patients with ischemic stroke in the previous study of the research group, combined with literature review and expert group discussion, the supportive care information platform version 1.0 was developed. After 5 patients with ischemic stroke hemiplegia tried for 3 weeks, it was optimized and upgraded to version 2.0 according to the use experience. From June to December 2020, 64 patients with acute ischemic stroke hemiplegia in the Department of Neurology of the First Affiliated Hospital of Soochow University and the Second Affiliated Hospital of Soochow University hospital were randomly divided into intervention group and control group, 32 cases in each group. Barthel Index and balance ability were compared between the two groups. The control group received supportive care plan, and the intervention group received supportive care information platform. Results:After the intervention, the scores of Barthel index in the intervention group were 85 (75,90), 95 (95,100) at 1 and 3 months of illness, which were significantly higher than 65 (55,75), 75 (70,85) in the control group ( P<0.05). After intervention, the scores of balance ability in intervention group were 37 (28,43), 48 (45,51) at 1 and 3 months after illness, which were significantly higher than 28 (16,36), 31(20,37) in control group ( P<0.05). After the intervention, the scores of stroke related knowledge in the intervention group one day before discharge, one month after illness and three months after illness were 14.97 ± 2.98, 19.00 ± 2.14, 22.81 ± 1.60, which were significantly higher than those in the control group (8.69 ± 1.64), (8.88 ± 1.52), (9.09 ± 1.61) ( t values were 109.37-1 169.84, P<0.05). Conclusions:The application of supportive care information platform can provide targeted nrusing measures for patients at different stages of disease development. The platform has good feasibility, and can significantly improve patients′ Barthel index, balance ability and stroke related knowledge, which shows that the platform has good feasihility and clinical application value.

ABSTRACT

Objective:To establish the Ultra-High Performance Liquid Chromatography (UPLC) characteristic chromatogram of different parts of Alpinia oxyphylla Miq., and to compare different parts of the chemical components based on multivariate statistical analysis. Methods:The UPLC was used to establish the fingerprint of Alpinia oxyphylla Miq. . The chromatograms were matched to generate the UPLC charactersistic chromatogram of different parts. Based on the variance analysis of single factor, combined with the Principal Component Analysis (PCA) ,Cluster Analysis (CA) and the Partial Orthogonal Least Square Discriminant Analysis (OPLS-DA) to analyze the differences of different medicinal parts of Alpinia oxyphylla Miq.. Results:16 common peaks of Alpinia oxyphylla Miq. were demarcated in crude drugs, compared with the medicinal materials of Alpinia oxyphylla Miq., the peak 13 (tectochhrysin) was lost in the decoction pieces, and the shell were missing peak 5 and peak 6. The results of PCA and CA showed that 15 batches of different medicinal parts of Alpinia oxyphylla Miq. can be broadly divided into 3 categories. The OPLS-DA result showed that the value of the peak area of peaks 14 (Nootkatone), 4, 7 and 12 were the main factors affecting the chemical composition of different parts of Alpinia oxyphylla Miq. .Conclusion:The fingerprint determination method established in this study is stable and controllable, which could distinguish the different parts of Alpinia oxyphylla Miq. .

ABSTRACT

Objective: to determine the levels of interleukin-6 [IL-6] and tumor necrosis factor-alpha [TNF-alpha] gene transcription and protein expression in peripheral blood mononuclear cells [PBMCs] of children with bacterial and viral infectious diarrhea

Study Design: an observational study

Place and Duration of Study: department of Pediatrics, Pingyi County People's Hospital of Shandong province, Pingyi, China, from January 2017 to February 2018

Methodology: children with bacterial diarrhea and children with viral diarrhea were assigned to respective bacterial and viral infection groups. The serum IL-6, TNF-alpha levels in the two groups were detected by ELISA. PBMCs were collected from study subjects, and mRNA transcription expression level of IL-6 and TNF-alpha in PBMCs of the two groups were determined by semi-quantitative reverse transcription-polymerase chain reaction [RT-PCR] technique, and IL-6 and TNF-alpha protein expression levels in PBMCs of the two groups were determined by Western blot analysis

Results: in a total of 50 children with bacterial diarrhea and 70 with viral diarrhea, the levels of serum IL-6, PBMCs IL-6 mRNA transcription and IL-6 protein expression were higher in the viral infection group than in the bacterial infection group [all p<0.001]. The level of serum TNF-alpha and PBMCs TNF-alpha mRNA transcription and TNF-alpha protein expression were higher in the bacterial infection group than in the viral infection group [all p<0.001]

Conclusion: when it is difficult to clinically identify the pathogeny of pediatric diarrhea infection, adjuvant diagnosis can be made by virtue of IL-6 and TNF-alpha changes in the blood to choose the correct treatment

ABSTRACT

Objective To observe the clinical effect of Ozagrel sodium with ultrasonic therapy in patients with acute cerebral infarction (ACI). Methods Eighty-three ACI patients were randomly divided into 2 groups: ultra-sortie therapy group(n=42) received Ozagrel sodium and ultrasonic therapy, whereas control group(n = 41) only ac-ecpted the Ozagrel sodium for injection. Both two groups were under conventional treatment. The level of laboratory in-dex and the neurological impairment were detected before and after treatment. And the clinical effect were detected in the end of the treatment. Results The neurological impairment and the clinical effect defected after therapy in both groups, had statistical difference(P < 0. 05), especially in the ultrasonic therapy group. Conclusion Ozagrel sodium with ultrasonic therapy in patients with ACI could promote recovery of the neurological function, thus increase clinical effect.
